I'm trying to add MOH to an old Strata DK16 but I'm confused looking at the manual.

It shows tel 4 OR BGM on the last pair of the 66 block. Presently that is their last voicemail port that does the signalling.

I must be interpreting this wrong so can anyone give me some clarity?

There is a RCA jack inside the box for MOH right beside the amphenol plug (J6). Plug it there and it should work by default except if the port is defect

I never would have found that. I saw no reference to it in the Manual.
