Its been a while since I've had to troubleshoot this. Has anyone ever found a definitive reason for why the SVMi8's do this? I never found a common reason for this, and I've had it happen on around a half dozen systems or so. The current one having an issue is an early OS500, something like 1.31 software. Only way to bring it back is to hard reset the card by reseating it in the slot, or even move it to a spare slot.
Any ideas?
