A customer of mine is having an issue with one of his extensions displaying "not in service". They have a MICS with a PRI and the extenisons DID doesn't ring either, it gives a fast busy. I checked the programming and everything is correct. The target line has been assigned to this extension and the extension in question does exist. Any ideas? Thank You.

You can try to Disable that extension firs and then Enable it again. Make sure it is not in DND mode. If DID rings busy it's might be a problem with provider or check out your target line programming for busy condition: Go to prime or busy signal.

The last time I encountered this, it was a bad phone.

Apparently the user can make outbound calls. However, when I went into the Maintenance setting of the programming, I found that the extension was showing "unequipped". This customer likes to utilize the set relocation, I'm wondering if that screwed up the extensions. I'm waiting to hear back from him if the extension number is the one he thinks it is. Thanks for the help!

Set relocation can definitely cause the system to act strange. I would turn it off, unless the customer has insisted on having it.
In which case I would inform the customer should they continue to use the relocation feature, they should expect to have to default the system and re-program it from time to time.
Set relocation is a terrible feature. IMO

I think you better verify that the phone in question has the DN you think it has. These are all symptoms of its ext xxx when it reality it is ext yyy.

Yes, Marv is correct. When in doubt, Feature *0 and press an Intercom button.
