Well, Toshiba doesn't have a clue, let's see what you guys come up with.
I've got a CTX670, 6 stations in a pickup group, all with PDN and PhDN keys and call pickup key. If a call comes in (internal or external) it rings the station and you can use the pickup key to answer it from another station. If the station that receives the call is in use, it gets a muted ring on the PhDN, but the pickup key won't answer it. Tried all options under call pickup and all #5#?? options as one touch, it either goes back to idle or says 'pickup busy'. Customer said it used to work. Any suggestions?

Are the incoming calls first transferred by auto attendant?

No AA. All outside calls are answered by the operator and transferred or are DID. It doesn't matter where the call originates from (operator transfer, DID, internal, outgoing...) if the phone is busy, you can't pickup a second call.

I have a question for you question. The above, muted ring on the PhDN, has me thinking. A call is transfered to ext 205 ext 205 is busy so it muted rings a PhDN on 205 and you try to pickup ext 205 is this correct? If so the call even thou it is ringing the phone 205 is really on the PhDN ext XXX so pickup would not be to 205 it should be XXX.

Check your pick up codes.......

CTX is #5#5XXX where XXX=estension number. #5XXX was the DK code for pick-up

SS - Tried picking up both the PDN and the PhDN with same results.
Meister - Checked everything relating to pickup in COS.
Voyager - Tried that along with #5#22 (pickup group) and everything else listed for pickup.

Customer said it's not that big of a deal, but I'd like to know what's going on anyway.
