J have an Adix where the system name only displays on 2 of the 12 digital phones. I can't see any difference in programming. I did change the system name (to the same name) and upload it, I then saw that changing that field (04.01) requires a reset. Set up an auto-reset that evening, per the customer it still doesn't display. Not a big deal bur I wonder if there is a setting to display the system name. These are gen 3 phones. On another system I noticed a gen 1 phone that displays the name of the phone and extension when idle, so maybe there is a setting? Haven't found one yet.

If your system is an ADIX, the display setting is global in Class 4.01 as you noted. You can test the display of the phone itself by pressing FEAT #, 9 at any phone, then press a button. The display will show how it's programmed, verifying the display is functioning. KTD-3 phone displays are field replaceable, and sometimes work themselves loose. Try unscrewing them, removing them and re-installing them. Unplug the phone first....
If your system is an ECS, all of the above troubleshooting applies, however there is programming that allows the phone to either display system names or station names. That's how the series 1 phone was displaying what it was....that's found under Stations>Display or Class 70.07.

The displays of the phones are fine, untess just the bottom row is defective, not there now so I can't test. Two new phones (ports) we installed do show the system name, I previously found the 10.70.07 setting in other system databases, then found that databases with the .sdt file name don't have 10.07.07, they do have .07.06 and 07.08. (those with .pbx do) The system in question is an Adix VS, so the 70.07 setting doesn't exist. I'll see if I can test with our shop Adix VS, maybe load the customer's db in that.

I've heard with that version the phones must be plugged in when the system is powered on to display the system name - and although the scheduled reset feature is available on the programmer it may be not on the VS release. The 2 new phones (all are V3) may have been the only ones plugged in upon power up, I don't know. Next time I'm there I'll need to see if I can do a reset.

The ADIX and VS versions don't have a way to show anything in the display other that what is programmed in Class 4. This is only available in ECS switches. If you change the settings in Class 4, you need to do a reset to make them show in the phones, and having the phones plugged in when powering up avoids all sorts of goofy little issues.

Tried that last week in the shop VS and that's what I saw. The VS needed to have the phones plugged in upon power up, but the Adix I'm building (to replace an Omega 3 system!) did not. As you said, best to have them plugged in.

Went to the site in question, after resetting the system all the phones now show the company name.
