First, I was able to receive inbound calls. I made some changes per advice I received and now I can make outbound calls, but now inbound goes to fail over number.
There are so many things that can go wrong with SIP trunks. Since the provider is forwarding to a fail over number then it makes me think that either a) the provider doesn't see the SIP trunk registered b) the Toshiba is rejecting the incoming call, or c) the CIX didn't respond to an Invite message. If the call was immediately routed then I would guess a or b. If there was a delay then it could be c.
Wireshark could help clear some of this up. Often the provider can see the SIP messages and should be able provide a little information. It may be good to learn how to run Wireshark properly. Sometimes the firewall can capture Ethernet packets.
