admin uses barge in feature extensively.
Admin is COS 2
All others COS 1
Getting a complaint that she can barge in on some but not others.
Display says barge in failed.
Can't see any diff in phones.

I have this same problem in a call center. I think I have narrowed it down to extensions that have been moved with set-relocate.
Is it consistently the same extensions? My customer says it moves around. He resets the system and it's fine for a while too.

A while ago I had a customer who's employees figured out they could not be barged in on if they were on a conference call so they would make a conference call with their cell phone and the intended party to get around it. Shows the same error of Barge in failed too.
