Found this add-on module in a never-opened box in storage. Basically an addition for the operator phones which adds another (you guessed it) 44 DSS speed dial keys.

Only oddity, it has only a single RJ11 jack and all of the operator phones I have also have only a single line jack (plus the headset jack).

So . . . how does this thing get connected and used? Y-cable?

another station port, you add it to what ever phone in programming. I think there was a limit on the # of ' add-on ' modules per system

It will use a port with standard 2 pair wiring. You will need to associate it in programming using Procomm so it knows what extension it is attached to.
This group never ceases to amaze me - this phone system hasn't been in production in more than a decade and still the responses come in almost immediately!

I was thinking this would connect to a second jack on the main operator phone. I'll see if there is a blank in the wall plate to add another 2 pair jack and make the run back to the box.

Thanks guys!
