Came in today and there was no dial tone on any of the handsets. I did some test calls and we are getting inbound and outbound service.

Can anyone tell me why we lost dial tone?

This is an InterTel Axxess 5.349 system with PRI T1.

These errors are showing up several times over the last two days. Help!!!
I went ahead and hit the reset on the CPU to see if I could get dial tone back. The cards all came back, as did the displays on all the phones, but I got no dial tone and no service whatsoever. Then, whammo... about a minute later the system shut itself down and came back up with Red and Yellow alarms (alarm #114 - T1 is in red alarm)... it did this twice on its own consecutively and finally came back up. Now everything is working normally... with dial tone.

I really don't know how to interpret this at all. Bad power supply? ...bad CPU? ...T1 service provider issue?

If you're using DKSC16+ cards, try resetting the one running the phones w/ this issue. If it resolves it you probably need a DSP upgrade on that card.
There are two DKSC16+ cards. What would be the procedure for resetting them? I don't see a reset switch. Should I just power down the system and take the cards out and reseat them?

By the way, the phone system has reset itself twice since my last posting... I think something is very very wrong.

I went ahead and ordered a power supply card off of Ebay just in case. I'd like to do the same for the CPU, but I'm not sure how to go about choosing the right CPU with the correct software version and chipset. There seems to be a lot of variation even among the same part number.
I must have been replying when you updated with the resets. Pressing the button effectively did the same thing as reseating the cards, but also call processing as well.

In the future you can reseat the DKSC16's by popping them loose using the white tabs at the top and bottom of the card. No need to power down for this, either.

As far as resets go, you could look in the other error logs to see the last time the system reset. If it was a while ago I wouldn't be too bothered by todays events, but I would do a backup.

In the future if you run into this try reseating the DKSC16+'s first. There is a DSP upgrade to correct no DT for older 16+ cards.
Thanks for the info, Jake. Though, I'm not sure the DKSC16 cards are the problem. Later, the system again reset itself twice... this time without any provocation from me. This was not part of a nightly maintenance schedule since it was at about 4:45 p.m. I'm having a hell of a time trying to determine what's causing these random hardware resets.
It's gotten worse and I still haven't been able to get a tech. out, due to weather and the fact that there's only 3 companies here in Denver that do InterTel and one of the two require maintenance agreements.

Now I'm seeing system alarms and resets (the whole PBX shuts down and turns itself back on) up to 6-7 times daily. I'm getting Illegal Command ID Sent To Board: [C6,01,00,08,12,13,04,80,1A,10,1E,1E] in the logs repeatedly. Can anyone help?

I have a new power supply that I'm going to install to see if that resolves anything as soon as folks get off the phones... this is a very busy small office. Thanks!
Oh... and one more thing. Now when we pick up the phone there's a loud buzz on the line. No dial tone... just a real loud buzz. When we dial out the buzz goes away and calls are clear. This just started happening today. Does any of this add up to anything in anyone's mind?
Replaced the power supply with a new one... the buzz went away and the phones are working, but they did reset themselves again five minutes after changing the power supply. I get an audible alarm and then the system goes dead and reboots... shows Alarm 114 upon restart and automatically clears the alarm. I have to assume at this point that the power supply is not the issue.
At this point you can do a database save and run
the database repair utility on the database you
saved and then upload it back to the system. If
you still encounter the problem you should upgrade from v.5.349 to 7.

Maybe, but unfortunately that won't be free as it would require buying a new PAL.

Don't worry about alarm 114, just the T1 synching back up and completely normal.

It's interesting you hear an audible alarm before it resets considering the system doesn't have one. I'm thinking battery backup. Is the system plugged into one? If so 86 it and allow run time.
There is an APC battery back-up/UPS attached to the system. I don't think it's malfunctioning (lights look good) but I'll connect it to just a regular surge protector just to eliminate the UPS as a possible culprit. Do you think based on the output from the Database Repair Util that the errors it fixed could have been causing the system to reset?
Honestly, no. You might have saved a little memory, but I doubt it solved it. This seems to me like something environmental.
I agree that it sounds like the environment. You best bet is if you can catch the pattern and be in the room at the same time as a reset to document exactly what occurs. Mainly looking for any of your power supply lights to go out or certain cards that go busy. One other thing you may try is to power down the system and reseat all of the cards to make sure there is a good connection. I have had issues with Techs screwing the cabinet down too tight and the backplane being off wack.
I had an Intertel tech. look at it last week and he couldn't find anything wrong...aside from us not getting name and number caller ID with our PRI/PAL ...which no one has ever been able to figure out (we just get number).

After the tech. left the system again went down (reset). I took a deep breath and made sure there were no baseball bats handy. I removed the phone system from the UPS we had it on. I think that must have been the problem. It's odd, because I have a PC attached to that UPS as well and it never goes down when the phone system does. But, I'm going to see how things go. If the system is up for two weeks straight, I'm going to conclude that it was the UPS causing the issue and I'll likely replace it. Why a UPS would cause the dial tone issues... I have no idea. I suppose it could have been underpowered or some such, or the resets were causing the PBX to not always come back up correctly. In any event, it will sometimes take a week before the resets start happening... so, I think two weeks is a good litmus.

Thanks to everyone who took the time to help with this issue. I learned a lot. I will post back after another week to let everyone know whether or not the resets continued. In the meantime, I procured a replacement power supply and CPU card... so, I think I'm pretty well covered if it turns out not to be environmental.
