I will state up front that I am relying on the customer to "accurately" give me the error info but here's what is going on...
Customer lost internet to the home and once internet was back up tried connecting the phone (5224). Phone starts to boot gets an IP address from the router and then goes to downloading and downloads up to 130 and quits with the HFTP Error 4... rebooting phone. It just continues in this loop.
I've had the customer default the phone and reprogram the TW gateway on the phone. I've deleted the set from both the TW and ICP and reprogrammed and the issue remains.
Any ideas?

Turns out it was something the customer's ISP was doing that caused this error. Once they had their stuff straight the phone synched up with no other problems. ISP = I Screw People?
