You know, I have actually seen what you are describing ALLN1 and I might have bought it in this case. The problem is that the Verizon tech double-punched the transmit pair's tip onto the ring of the POTS line on the pair directly above. There's no way that this wouldn't have caused a huge noise problem on the pair.

The bottom line is that the Verizon installer probably tested it in the basement on their NIU, threw in a few quick cross connects for the smart jack extension, then headed for the liquor store. It happens all the time around here. All I can say is that I am glad that the customer's representative (pretty technical) witnessed what was wrong and saw us correct Verizon's T&M work.
