Is it typical for carriers to provide the outbound caller ID on a PRI by default unless they are told that info will be provided by the caller's PBX?

Yes, they typically send the billing telephone number for the account unless told to send something different. They can also configure the circuit so that the connected PBX sends the appropriate number. Note that the name cannot be altered; only the number.
