It has been almost two weeks since my last post above so I take it no one knows or cares. Thanks to a kind member of TCI, I was able to get it working. The problem was a defective 108 speaker which is unusual to the point of wierd because I had tested it several times with my usual 4A speakerphone setup and it worked fine.
I will go ahead and answer the questions I posed above. I did move the green wire from Terminal #8 to #16. Then, all one has to do is connect the following wires from the M16C cord to the following locations on the terminal board: W-BR to 10, W-O to 22, BL-W to 6, W-BL to 25, G-W to 24, W-G to 30, and O-W to 29. Additionally, place W-S to G and S-W to A on the network. The other wires from the M16C cord are insulated and stored, and with my cord, they already were.
