You can verify by removing the card and visually inspecting. On a Digital, bay 2 - slot 3 is lower right, upper level on left. What else is on the universal card? Also what type of card is in bay 2 slot 4?

If we are in agreement, the 600 Ohm output of your music source needs to be connected to pair 23(V/G) of the bay 2 slot 3&4 block.
The module only provides an interface to an external source.
My question regarding slot 4 is if there is a T1, they may have failed to recconnect the cable.

Sorry but I'm on the same point: MOH doesn't work. MOH module is on Univ Card (module # 4). Option # 602 is ENABLE in all COS in use (rooms, trunks, digital phones, console, ...) now.
I took a look on cards and they are as follow:
Bay=02 Slot=03 UNIVERSAL CARD 02-03-01 CONSOLE MODULE 02-03-02 DTMF RECEIVER 02-03-03 DTMF RECEIVER 02-03-04 MUSIC PAGER MODULE (Music/Paging Module 9109-018-000)
Bay=02 Slot=04 UNIVERSAL CARD 02-04-01 CONSOLE MODULE 02-04-02 DTMF RECEIVER 02-04-03 Non programmed 02-04-04 Non programmed
I punched down on pair 23 of J25, MOH amplifier 600 ohm output, I'm able to hear it but several calls on hold and I just hear air".
Any other idea to checking, please.

Could be a bad card, module, or cable. You could move the module to either the 3rd position on the same card or positions 3 or 4 on the slot 4 card.
You would need to make the changes in form 1 and 18

Testing Music-Pager-Module in different UNIV card now. 02-04-04 circuit corresponds to pin 10 (RED/SLAVE pair) on same J25 connector. Is this correct? Thanks.

Red/Slate pair would be correct on same J25 i.e. 10th pair.

Sorry guys, but no good luck so far. I installed same Music/Paging Module hardware on a different MITEL SX-200 and it worked. Also, I moved it to 02-04-03 and 02-04-04, with setting on form # 1 & 18, which implies different UNIV card and different pairs although same amphenol cable, and nothing good so far. Any other suggestion? some option to set we're missing? Thanks!

Have you checked/changed the AMP cable - or can you move it to the other bay to eliminate the cable?
And as JohnP stated have you verified you input impedance 600Oohms and input level -6 dBm. Its been a while but I remember installing an inline matching transformer to get MOH to work on a 200D.

2-4-3 and 2-4-4 are the same card different module which would be red/orange or red/slate pairs of J25. Are those what you are using? Do you have 2 consoles and do they work?
