I have a DK424 installed using one PL key per phone. I have 2 out of 17 phones where the PL will not flash on an incoming call. I have all the phones programmed to ring in program 81 and FB 12 in program 31 is off for all ports (this was on and the PL button wasn't flashing).
What am I missing here?

If prog 40 is OK, verify the pooled line group key in prog 39 (3XX).

Program 40?? I don't have that in the program sheets - they go from 39 to *40.
To answer the questions, yes, you can dial out from all the PL keys. At this point all the PL buttons on all the phones light when pressed. There is one phoen on port 000 that when t he PL button is press I get dial tone, but no LED. This is the same with Line 5 that is programmed on the same phone. I have tried putting another phone there 9that I know works) and I get the same result.
What else??

In program 39 make sure that there are no line appearances other than the pool lines. For example, if you have line 5 on button 11 of a 10-button phone, the call might be hopping onto that button but you can't see that because you have a 10-button phone.

All phones have 1 PL key (301) on button 2 and Line 5 (005) on button 3. There is no programming on buttons 11-20 (I verified that). All the phones work OK, meaning the PL button and the Line 5 button will light when a call is ringing in or when the button is pressed, EXCEPT the phone on port 000. Other feature buttons do work on this port (DND, phantom for the door phone, the door phone unlock button)
I have verified that all ports (000*031) are programmed the same in programs 30, 31 and 35 and they are all set to ring for lines 1-4.
I am perplexed!

Even if not using DSS consoles or Add on Modules, check and make sure there are no CO lines programed to appear on those keys (Prog 29 and Prog *29). As phonemeister said, it appears the LED indication is hopping off the PL key and the CO line 5 key.

What would cause these LED's to not light up when they are pressed to get dial tone, where other LED's on the phoen light-up fine.
There is one DADM on another extension that does have the CO line appearance; however, the problem I am having with port 000 was happening BEFORE I installed the DADM so they aren't related.
I should say that this system was moved from one office to another and it did have a DADM on the main port. I will check to see if there is a DADM still on port 000.

check P*29 for port 000 and see if you have any line keys programmed as this will cause the PL key not to light up of flash.
