sounds like a bad card possibly?
Look at the card, is it an ESIB/E U20?
If so there should be a jumper to make it a U10.
The U20 card did not work in the IPK switch without moving the jumper.

Okay, that would make sense. I'm 90% sure that someone screwed with the switch between the last time it was used and the vendor taking it off the wall, so that makes perfect sense.

Yeah, wound up taking all of the cards out, one by one, dis-assembling the ESIB, inspecting and cleaning all, then reassembling. I was going to try one more bright idea on the ESIB card, but when I put all back together, it fired up and works as designed.
I'm guessing that the daughter-board on the ESIB was poorly seated. Either that, or GSW/PFM.
