The only time you use a crossover on a T-carrier circuit is when you are going from one "smart" device to another "smart" device. When you go from a "smart" device to a "dumb" device you use a straight through cable.
A "smart" device to me, is a device that creates a T-carrier signal (i.e. a mux, a microwave radio, a channel bank etc.). A "dumb" device is a device that does NOT produce a T-carrier signal (i.e. a patch panel, a smart jack, an RJ-48x etc.).

Most people around here just run one 4 pair cable for T1s and PRI and use a normal cat 5 patch cord when extending a demarc from the smartjack into a suite. Not the best way to do it, so I've heard.
