I wouldn't be surprised if something was still strange on the pairgain. I went out with the VOM, Linemaster, and butt set, the voltages looked fine and I still had battery during the ring cycle.

Is the IPO grounded?
Also have a customer, way away from civilization, lines would not ring in when on the ATM-4 on an IPO 406 control unit. Would work fine when run into the former Partner system, and mapped to ring through to extensions that were then connected to the ATM-4.
Sandman's ring extenders did nothing.
ATM-16 module works fine.

Receptionist picks up phone and it stops ringing, but caller still hears uninterrupted ringing. Do you really mean UNinterrupted ringing? Or do you mean that the interrupted ringing continues, un-tripped? If you put a ringer across T & R during this sequence of events, does the ringer continue to ring, following the CO ringing cadence of 2 seconds on, 4 seconds off?

I think it's both the IPO and the pairgain. The IPO off hook resistance is probably higher than normal but is right on the edge- just enough to trip the ring on the other lines. The pairgain is on the edge the other way, requiring a "low but normal" off hook resistance. The IPO will work fine with the other lines and the pairgain line will work fine with other equipment like a buttset.
Just out of curiosity, could you measure the off hook line current with a buttset connected to the pairgain line then the IPO. Compare readings. I'm willing to bet that the IPO will draw less current.

I'll give that a try next time I'm out there. Avaya is trying to find a tech of their own to send out but they've let so many go that I think they are looking in KCMO or the twin cities (both 3-4 hour drives from here).
