Go thru all 48 ports several times. Check to see if the problem is on the same port or if the problem is at ramdom.( your looking for static or popping noise) What cabinet are the T1 cards in? What cabinet are the SLT's for the modems? If they are in different cabinets, check the LAN screen for errors.
Good luck

OK, i have to assume that ports 48 and 49 are on the same T1 board, same cabinet. correct? DTMF receivers in the same cabinet. correct? I rarely have seen a problem with the T1 circuits where they would have individual problems per channel. it's usually all or none. you are setup as D4 and AMI? correct? Line build out is set at? hopefully 0dB, don't pay attention to that garbage in some of the older manuals about using -15dB. you seem to have enough DTMF receivers. If you have the system backed up, try defaulting it and then retry your application. If the problem still exists, then try changing the EVCM. I keep going back to the EVCM because the time slots were on that board, it's possible that something is defective. you understand that there are some limitions using modems through the system. they will work but you got to play with them. some modem manufacturers worked better than others. with modems, I usually got better results using T1 than the copper circuits.
