I have options 71 & 72 set to 2 in Form 4
Form 18 is programmed
Nothing Programmed in Form 19
MOH works internally

When connected through PRI any call put on hold gets a loud high pitched whine.
Have replaced the following:
MCC
BCC II
IAD2400

Not sure where to go from here. Customer has their customers hanging up when placed on hold because they think they are getting transferred to a fax or because it's just that damn annoying.

may not be applicable but here this is from a Mitel product bulletin:

Options 71 and 72 must correspond to the type of FIM or CIM card installed. A triple FIM card can be identified by the part number [see below] or the factory mounted fiber connections on the front of the card. A triple FIM has 4 fiber connections on the front edge and a connection socket on the rear for the third bay. Even if the extra module is not installed, this is still a triple FIM. For the triple FIM card, Option 71 or 72 must be set to 3 to correctly assign the links.

Control triple CIM cards have a dip switch [switch 4] that must match the programming in Option 71 or 72.

PRI cards installed in the control cabinet, emulating either bay 2 or 4, must have Option 71 or option 72 set for 2.

The Triple FIM part numbers are as follows:

9109-613-001-NA
9109-613-002-NA
9109-613-003-NA

How is the PRI programmed, bay, slot etc.? Is it in the Main Control cabinet?

Main control cabinet slot 11 just one circuit so bay 4 slot 6 ccts 1-23
