FQDN
Set up a FQDN, there are a few free ones on the internet. Or, if you have access to your website, make a subdomain and forward that.

Once that part is out of the way, you forward your ports needed. 8444 is for the app, you need 5222 for Lync integration (8.1) and 5269 for the chat federation.

BOOM! Done!
