We're having a decidely weird problem with phones locking up at one site. The system is a mitel SX-200 EL/ML running a 2001 version of LW (I need to have someone onsite double check the version)
Both the 4050 and 4025 phones will occasionally freeze. The time on the display will go static, and no dialtone will be heard on the phone. Unplugging and replugging the phone from the wall will fix the issue. It happens to single phones at a time (not every phone on a card or bay) but it has happened to more than one extension.
So far every phone that has locked up is on the same Digital card... but there are only a few users using the other cards, so it might just not have been noticed.
I've got plenty of spare digital cards, so that is probably my first replacement, but has anyone seen or heard of this issue before?

No fix yet . This has happened on and off since the SS420 was first released and has continued through the SS4025/4150 series phones .

i've seen it too and now make it a practice to de-program any DNIC ports that are programmed with no phone attached turn off background diagnostics and I regularly clear my errors and logs.

Talked to tech spt. about a similar problem. We have a site where DNIC cards in a couple of slots keep locking up. Blank screen on all phones from that card.
Nothing in the log. Reseating DNIC card clears problem. According to Tech Spt., most likely cause is static charge generated by users with headsets. When they sit down and plug in, it zaps the DNIC card.
Site has fresh carpet and many users on headsets. If your site is similar, you may have a static site.

I've seen this many times too, on Sx 2000 with multilines phones , expecialy ss430 , and we have no headset users at all. Also, try to see what hapens with a SS401 if you disconect his line (unplug it) during a call ! The PLID often remains locked in that call, even the SS401 isn't conected anymore! Also try to see what hapens with a hard hold after you reset the phone (you unplug and replug the phone) - with this bug our users can leave calls outside to outside PBX . The only recomended thing (that you probably know already) is to distribute uniform the multiline phones on DNI card (not having too many on the same card) and to have the consoles on card with no multilines phones at all. In rest expect from Mitel to solve some bugs !!

update your software to he latest release Q45 19.3. It has better logs so you can diagnose the problem, easier.
I have found that one bad phone can take a card down, one phone at a time. If you wait long enough every phone on the card will go down.
