If you got the problem at the MDF with the Cross-connect lifted, the problem is internal. It sounds like it happens randomly to sets on various cards.
Does it always happen to certain phones, and never to others, or is it equally likely to happen to any given phone?
If it's only certain phones: An option is set for those phones which is not set for others. Check COS, etc. and compare a good set to a bad set in programming.
If it may happen on any phone: A global (system) option is set.
Has this always happened, or just recently? If recently, after a change of some sort, or without any changes?
If it's recently, after a change, reverse the changes and see if it clears. If it's recently, but not after a change, check the processor.
If it's been this way since installation, check processor and if possible reprogram from scratch.

all sets are in the same COS this problem will act on several phones then dissapear for a bit of time
just recently last 6 to 7 months , but no changes have been made to the COS Switch is redundant and problems have occured while running on both A and B plane
have been logging when the problems occur will monitor problems for more information

Possibly a flaky power supply.
A flaky PC power supply will sometimes cause GSW (General System Weirdness).
