Dead ports are not all on same module. I tried the disable/reenable but did nothing. I'm thinking maybe when software was upgraded there was a problem.

8x24? with only three dead ports and 70 stations? your lucky!!
That thing is old. A reset would get rid of the wierd looking software issues but the hardware problems sound normal. you could get replacements on the cheap or look for free ports if you have any.

A few dead station ports would be OK but it appears now when a phone is unplugged or the system is reset more ports die
