Total of 3. You, another inside extension, and an outside party. OR - you, and two outside parties.
If you use a phone conferencing service, then you're limited only by the service. Usually quite high.

You can "daisy-chain" conference calls with other users on the system, limited only to the number of channels/lines and extensions/people available.

But keep in mind that the volume will go down with each caller added.
