Do you have a problem in the U.S. and/or Canada with people not understanding which physical cellular network they are actually being served by?

Here in the U.K. there are only five actual networks. Hutchison is the Johnny-come-lately of the bunch and only operates 3G (2100MHz) services.

The established GSM900/1800 networks are provided by only four companies: Vodafone, O2, T-Mobile U.K., and Orange. The dozens of companies which sell packages (right down to Tesco, a national supermarket chain) sub-contract to one of these four networks, but often don't make it clear which network will be used, something which can be a particular problem out here in rural areas where coverage varies considerably from one network to another.

I've heard of people being told outright lies from places which claim to "have our own completely separate network."

Obviously there are many more cellular networks in the States and many which operate only on a local level, but do you find similar situations of sub-contracting where the customer isn't aware of which physical network is actually providing the service?

I havent but thats not saying much, the ones that stand out to me that could possibly be the way you are describing are pocket and cricket. They are limited in the area they cover but again I can not say they are trying to bs their customers in to buying something they dont cover.

The only problem here is the usual dodo that doesn't know or care...hey they pick up the phone & it works...that all they care...

Actually the real problem are the Big Ones who claim that their service is more coverage that the terrain permits...dead zones, grey zones, etc.
