Swap out the phones that are having the disconnects with the phones from a non-operator group. If the problem follows the phone then you know it's the phone. If the problem stays with the extension, then it's probably a wiring or cabling issue.

You stated previously that the call maybe already in progress before the call is dropped.
If this is a correct statement then I see two possibilities, one being a trunk line/card issue and two being a hookswitch issue with one of the 5 telephones answering the Operator calls.
So start swapping them out one at a time with known good working phones or swap out all five at once.

Sounds like a bad hookswitch to me. Since it's a 424, you probably have 2000 series phones, they have a known problem with the hookswitch and can cause disconnects.
