for some reason they arent on the phone but their DSS keys keeps on showing green on the Operator phone ( i have 2 modules) 40 exts but also if the operator is on the phone someone dials her she hears what they say (while shes on the phone) then it cuts the caller off of the phone..

Ususally a green LED on a DSS means they are talking to that extension If they can hear the other extension, maybe they called the operator, while the operator is on the phone and Off-hook call announce (OCA) is active.
I would give the operator it's own COS and dissable Can recieve OCA.

HIT:The lights on the DSS will stay green if no phone is plugged in to that extension.
One should not increase, beyond what is necessary, the number of entities required to explain anything.

They do stay lit, but actually it lights up red, like they are on the phone.

Hello Ritztech welcome to the board!
It sounds like you're using two add-on modules. Which system are you using (CIX or DK series)? Also, is the operator's phone "off-hook" or on speaker, when these LED's are green? It may be a bad ADM. If so, do you have any spares that you could switch it with?
