On a Toshiba Strata DK96, it is set up as a 'key' system where they just push line buttons to call out. Qwest put in a Cisco router which supplies a POTS channel bank. Half the lines on channel bank show 44 volts and half show 54 volts. One of the 54v lines is on phone system. Qwest monitored the call I attempted and they saw a 1 dialed when I didn't do any dialing. When I go off hook I don't even hear dial tone. Help.

You said when you go off hook you don't hear dialtone, does it light up the line key on the phone? does it show what line you are using in the display? What happens when you call the line?

Double check the jumper wire and make sure it's punched down correctly.

Yes the button lights up. The display name is right. And incoming calls are fine.

Going off-hook is being seen as a '1' by the channel bank. There is a bounce in the seizure process. Just listen to the pair with a buttset when the key system goes off-hook. You'll here a 'stutter-click'. Compare the sound to a different port that works and you'll understand what I'm saying.
As for half 44V and half 54V, that is 2 programming snafu's. Tell Quest you want a channel bank that puts out 46-48 volts from channel 1 to 24. Fix it or replace it!

Are you using PCOU line cards?? PCOU line cards are not compatible with channel banks. Only RCOU, RCOU/RCOS line cards are compatible. I learned this the hard way.
