Anyone every heard of a CTX28 randomly calling 911 on the first outgoing line.
Happens at a customer site about once every 3 weeks. Always on the first outgoing line. Sometimes it triggers the E911, most of the time it just picks up the line and they get called back from the police department.

Is there anything else connected to line 1, from the D-mark to the CO input on the telephone system? - Fire Alarm
- Security
- Et cetera
That would be something to look into, if you haven't already.

There is one possibilty. Tho I would think it would happen a lot more often then every 3 weeks.
Mailbox 991, part of the auto attendant, by default has DND on and extension field is blank. If the mailbox was reset or deleted and recreated, then it is possibly the voicemail dials 911 when a callers gets the auto attendant.
The first 9 picks a line for LCR. Then 91 is dialed then the last 1 is added by the voicemail to connect to the extension.

Nothing else on the line except for a fax machine, but the fax is running through the system on a single line port. Checking into that to see if thats causing it but maybe mis-dialing or what not.
I'll check the mailbox, thanks for the idea.
The line is line 4 in the system, and rings the SLT on incoming but is the first line in the huntorder for outgoing.

I've had to respond to 911 calls which turned out to be a type programmed into the fax machine. They mis-dialed the Oklahoma area code as 911 rather then 918. So I'd look for a typo, don't forget the notify programming in all of the mail boxes.
