I need to install a PRIB card in a triad 3 with FP-4 Pt# 9031-32 can this card be connected directly to the "Smart Jack" or do I need additional equipment inbetween such as a CSU. I put one is years ago and don't remember. If a CSU is needed please reccomend and any other important information

Yes, you'll need a CSU. An Adtran CSU ACE should be fine, unless you need to split out data.
I'm sure Ed will be along shortly to give you some further information and/or correct me if I'm wrong.

After looking into this further I am starting to panic. Setting up the Triad looks easy but getting the PRI up and running looks scary. I think I am gonna need some expert help.

It's not bad at all. Honestly, it comes up working on it's own for 99% of the installations. Yes, you will definitely need a CSU/DSU. The Adtran unit that NFC mentioned is the one that you'll need. We will be able to help you with it otherwise.

Thanks much Ed and NFC. I have ordered the Adtran T-1 ACE CSU(only). Now so that I wont have to spin or re-invent the wheel can you guys guide me on connecting the CSU to the PRI and the CSU to the Smart Jack? What is the pin-out or what type dat cable is needed - Standard 568b or a cross over or what and I will need a db15 to rj45 - what is the configuration of this adaptor ?

In most cases, it's just a straight-through 568B cable from the smart jack to the CSU. It all depends if you dial tone provider wires the RJ48X jack properly. If straight-through doesn't work, you'll need to reverse the transmit/receive pairs at the jack. I know that our carrier's channel bank requires a reversed cable. Remember that we aren't talking about a traditional network crossover cable. If a reversal is required, change the pairs in the smart jack first to make sure.
As far as the cable from the CSU to the PRI card:
Card pin #1 goes to CSU pin #2 Card pin #3 goes to CSU pin #5 Card pin #9 goes to CSU pin #1 Card pin #11 goes to CSU pin #4
Bear in mind that the LED's on the Adtran unit will tell you if the wiring is good. Also, remember that the PRI card takes the better part of a minute to fully sync with the circuit. Make sure that you set the card's clock switch so that it receives timing from the carrier. You don't need to do anything with the small ribbon cable that comes with the card, but leave it inside the KSU in case another PRI or T1 card is added later. When it's correctly in sync, only the bottom LED on the PRI card should be lit.
