All of a sudden the auto attendant does not seem to be working. All calls go directly to the operator. How do I get this up and running again?

I think I fixed it. The VM was not working either, can't have that, so I rebooted the entire system (should have done that in the first place) and all seems OK.

Whats up with #### ?. If you don't want to use VMID don't preset forward any lines to 440. Use 450 instead. If you use 440 and blank out the VMID, you are not programming the system according to its design.
