PRI requires that the complete number be sent in the call setup message. The delay is required to gather all of the outgoing digits before the number is sent. Since you don't know how many digits will be dialed, the system needs to wait for a few seconds to make sure the caller is done dialing. You can also dial a # after the number to start the call immediately.
Not sure what the problem is with DTMF. I'll let you know if we have any ideas.

this isn't a call setup problem. this is after the call has left the system.

OK, then how do they experience a delay? Ringback?

As for the Delay on outgoing it is a requirement as DSX Deus says. You can lower the interdigit timer to 3 or 4 seconds if you would like. This can cause issues if you have a customer that is slow in dialing though.
Another thing you may consider is having the customer dial # at the end of there dialing and the call will complete immediately
As for the DTMF Tones not coming thru I cannot think of a programming reason. I would start with the common things. Check to make sure you are set for slave on the clock. Check your line buildout is correct and think about putting in a CSU if you don't have one already.
