As mentioned by IPKII, what slot is the COIB card in??
It will only work in a 192 in slots 1-4.

Well, I guess it's time to try swapping out hardware. Try another COIB-8-U30 card if you have one. Then I would either try swapping out the CPU or try de-faulting it.
