I would Back Up the Database to your laptop, Pull all Card's from KSU, Default CPU, Plug card's in one at a time and see if your line's come up. If they do, it's your Database, if not it's your COI,(never liked those 4 port COI's)

My bad,thought it was an COI-4, Have you tried to swap anphinol tails? Could be a bad tail.
