We have Intramail answering our lines for prompts and from time to time we seem to be caught up in an endless loop when someone calls into the office. They eventually get a person after pressing their extension multiple times. It loops back to the main greeting.

I would check programming on the voice mail and ALSO what type of lines you are using in the system. Unfortunately some cell phone providers have AWFUL DTMF or MF and teh system can't understand what it is.

I checked the programming and it seems fine. It doesn't happen all the time - very intermittent. The incoming calls are coming from LAND line. We are running straight CO lines.

A digit-grabber might help.
Also, on some connections, the audio is one-way for a couple of seconds, and so leading digits may be lost.
I suggest extensive testing, duplicating the conditions as closely as possible.
