My OS7200 is randomly transferring active calls to the operator group. Right in the middle of a call. It has happened to multiple extensions. Has anyone ever seen this happen?

Are you on a pri?
I have similar issue with a 7400 except calls are being "switched" to other people in the company without warning in the middle of the call.
