Setup a test dial block to transfer calls to a cell phone, wouldn't work using either Centrex Transfer or Off Premise, but was able to get it to work using On Premise and 9,xxxxxxx. Is it likely I need to contact the CO before Off Premise or Centrex would work?

No, sounds like you need to check your off premise station block to make sure its dialling the 9, or not.

Not sure on centrex

Also try setting supervision level to "partial" or "full" and test it. Have had similar issues, and this fixed it.
