I'm sure someone will chime in with a better source, but I know Black Box will make custom cables like this.

I buy all of my pre-terminated fiber cables from CSC (Communications Supply Corp.). Their number for the local branch is 301-353-1150 or www.gocsc.com. I usually get them within a week, perfectly packaged and they work every time. I like the fact that you can tell them the exact length, the connector types, etc. It costs more, but on a small job, it's a lot easier than risking field termination error issues.

Any of the major distributors (Graybar, Anixter, CSC) can handle this custom assembly.

However, allow me to recommend that you use 50/125 instead of 62.5/125 (unless you are augmenting a legacy system).

50/125 (OM2) has over twice the bandwidth of 62.5 (OM1) fiber and isn't any more expensive.

The only advantage to 62.5 was back in the old 10Mb days where you wanted to push the signal to 2km.

I do have to agree with HJB, 62.5 is the cat 5 of fiber. If you are going to install a new run of fiber and are not going to connect to or cross over to any 62.5, then I would put in 50 mic.
