Panduit has a one port Mini-com surface housing that is pretty darn small. The only problem is you have to use their jacks (keystone jacks won't work)

Systimax also has a single port enclosure, but it too requires the use of their jacks. Suttle Apparatus might have something.

The one you have pictured is about as small as they get. If you need smaller just use the jack without a surface box there are some that don't look to bad.

Leviton makes a single port surface mount box. Keystones should work in them.
