I am installing a CS-100 behind a Nortel PBX.
They are using the Nortel analog dial tone for CO Lines in the 100. So to dial out you press the Co line key and dial 9 to get past the Nortel to the real world. My problem is the have 4 digit extention numbers on the Nortel that they are networked to, can I program the 4 digit EXT # as a speed dial feature key? Or will they have to press the CO line key get Dial tone from the Nortel and dial the 4 digit EXT #?

I don't see why not. Put your "lines" (extensions from the Nortel) in the same line group. The key is, don't use Centrex/PBX Access Code (where the ESI dials the 9 for you). Then, just program your speed dial as you normally would, as 91234 (9 to get analog dial tone from the Nortel, and 1234 for extension 1234). Just keep in mind that doing so will then require users to press 9 twice to call out, or press a "line" and then dial 9.

That worked with no problem. For outside calls (long distance and local) I programmed the numbers in System Speed Dial with pauses.
