I have a customer that is using a 128 with one Remote IP phone. Recently the customer has modified their network and the location where the IP Phone resides is now a VPN connection and of course the IP Phone no longer works now. What changes need to be made if any.
Do we need a Local IP Phone to use in this scenario?

The target address of the Remote phone will have to be changed to the local address of the IVC card. ESI -Address onsite is the best way. It still has to be a Remote phone because even though it's on a VPN now, it still needs to "look through" the gateway to find the PBX, and it needs a internet gateway entry in its programming to know to do that. Only remote phones a allow you to program an Internet gateway in the F31 (or ESI-Address) parameters.
If there's only one remote phone at that location you can use ESI-Address from anywhere on the local network, so you may be able to help the customer through it without a site visit.
