I have a ESI switch that keeps having the same problem over and over. I have changed out the line cards twice and even changed out the entire system with a new unit and the problem returns after about 3 to 6 months. I added loop current regulators, new surge protection and grounded the cabinet and none of that made any difference. Any ideas on what the cause could be and what to try?

It could be a carrier problem using a softswitch.
