Is it mandatory that you have this specific card for the music source? The reason that I ask is that any audio source can be used with San/Bar systems, not just that card. If you are simply looking to provide a music source into San/Bar line cards, there are other ways to solve this problem.
The FM radio card was very expensive and therefore, not very popular. They will be very hard to find. We can probably help you use an alternative.

I have one of those in a San-Bar 6630 KSU card file. I can't find the practice right now, but it needs 3&18 or some such pins cleared for MOH.
Sit down Ed, because the list price for a 4201 card was $60! Not pricey by today's Premier standards eh?
About half that plus shipping probably gets it for the aggravation factor.
