Sure, I'll help you:
1. On the front edge of the key strip in the phones, locate the screws directly below each line button. Remove the far-right hand screw. Be sure to tape it somewhere inside the phone so it isn't lost. Removing this screw converts the last button to non-locking and it will be used as your signal key.
2. You will be placing the 401A card in the fourth line card slot. You will need to make sure that this slot has 24 volt "A" battery and ground on pins 3 and 18.
Be sure to disconnect any music on hold wiring from these pins! Do not install the card until all wiring is complete!
3. Connect the red/slate pair to the fourth set of station T/R clips on the 66 block from both phones.
4. Connect the black/blue pair to the fourth set of station A/A1 clips on the 66 block from both phones.
5. Connect the black/orange pair to the fourth set of station LG/L clips on the 66 block from both phones.
6. Install buzzers in each phone an connect them to the yellow/orange pair. Place the mounting leg under one of the screws that secures the dial.
7. Connect the orange/yellow wire from both phones to the "LB" or "MB" clip on the 66 block.
8. Connect the black/brown wire from phone "A" to the yellow/orange wire from phone "B".*
9. Connect the black/brown wire from phone "B" to the yellow/orange wire from phone "A".*
* You will need to locate a blind terminal on the 66 block or just splice these wires together using a Scotchlok connector (or something similar).
Operation:
1. Press line key #4 and go off-hook. Talk battery will be heard and the line key will light.
2. Press line key #5 while off-hook to buzz the other phone.
Operation is the same both ways.
NOTE: The volume of the buzzers is adjusted by tightening or loosening the center screw. You may want to test these prior to putting the covers back on the phones.

It sounds harder then it is.
Basically you're plugging a card in and connecting both stations to it. (Steps 1-5)
Then you're installing the equivalent of a doorbell in the phones (steps 6-7)
Then you're turning the last key on the phone into a doorbell button. (steps 8-9)

AG and AB used to be on pins 3 and 18. I took them off when I tried to install MOH. If I ground both 3 and 18, where do I connect AB?

I followed the instructions to the letter, but nothing happened. The thing is, I'm doing this on 2564's, and the buzzers were already connected to the yellow/green pair.
Should I leave them like that (rather, redo them), and then connect the brown/black's to the yellow greens?

You need to have AB connected to pin 3 of the card slot and AG connected to pin 18. Polarity really isn't important and locating AG/AB is fairly easy since it's stamped on the 66 block in the lower-right quadrant.
You had mentioned that you had 2564's which don't normally have a yellow/green pair. That's why I suggested the yellow/orange pair. Either one will work; there are no rules as to which cable pair is used for the buzzer. Either move the buzzers to the other pair or the connection with the black/brown wire. It's your choice. I'd put the buzzers on the yellow/orange if I were you.
Did the fourth button at least respond and light when you went off-hook? Unless you have AB/AG connected to pins 3/18, you aren't going to get much response.
