It's been a few years (decades?)I think 9 lines was the max on the 1st 25 pair and 10 on each additional 25pr. 50 pr phones were rather common but I have also installed 100 pr cables with 4 amphenols for the Attendant. One tech I worked with said the sleeves looked like Martian mittens (4 fingers).

That is correct, Dave for the standard 10/20/30 button sets. On the Call Directors, they used six button key strips with a "finger" of 20 pairs for each one (five rows of six buttons each). The most buttons I've ever seen on a standard 1A2 phone is 30.

Plant Equipment made 100 button Call Directors that were straight 1A2.
I worked at Doyle, Dane & Bernbach (Advertising Agency) on Madison Avenue here in NYC back in the '80s. They had a 1,000 line GTE Omni PBX and 1A2 equipment everywhere. The basic scenario was this:
An executive had a 6 button set, with, for example Ext 200, 201 and an Intercom Next door another executive has 202, 203 and an Intercom. Their (shared) secretary has 200, 201, 202, 203 and the Intercom on her 6 button set. Repeat this scenario with the next two guys and their secretary (204, 205, 206 & 207). Now there's a gal who covers the 4 of them (when one of the secretaries is out). She's got a 10 button set picking up 200 through 207 and the Intercom. Repeat with next group (208-215) NOW there's a gal with a 20 button set picking up 200-215 and an intercom. Depending on the size of the floor the receptionist had either a 50 button or a 100 button Plant Key set. She picked up all the lines on the floor and all the different Intercoms. Her phone was always ringing.
And did I mention that they were always reconfiguring lines and groups as they picked up and lost advertising accounts? What a job! Two men, full time, always doing 1A2.
The first installer that did the work was a dog. His work was was neat, but he hung external bells up. (Imagine 18 E1A ringers on a backboard!) I almost threw up when I saw that. I ripped them out and installed a diode matrix.
There were giant key sets called "Traders Turrets" that were mostly used by the Wall Street Stock, Bond and Commodity Traders and they might go to 200 lines or larger, but they (usually) weren't true 1A2. The Plant Equipment sets were true 1A2 though.
