Have you checked the SPI firewall in the router in CA?

My buddy took IP phones from LA and they worked (still) in Israel so it shouldn't be the phones.
That said I am not an IP guru. If they worked in England they shouldn't need a software upgrade.
As SS says, there almost has to be something preventing the phones from "talking" to the host. While we have a lot of good ideas here, maybe if you have no IP person there, it would be worthwhile to call in a Toshiba tech because trying to troubleshoot IP problems like this with no IT person familiar with Toshiba onsite is nigh impossible.
Sometimes you just have to call a Toshiba tech. What city are they in?

Also there is some AT&T U-Verse routers that are a problem it is based on their router and you would need to supply your own and place theirs in bridge mode to operate.

In CA - As stated on an earler post, you need to make sure any ALG settings for SIP or VoIP are disabled. Many of the routers will have it enabled by default. You will usually find this in the firewall settings.
There are a few models of routers that simply will not work. The Linksys WRT54G is one that I have personally come across.
Not sure how your DSL is set up in CA but is it possible to connect just the IP phone to the DSL modem and see if it works? The phone should be able to pull DHCP if that is how your circuit is set up. If you have a static IP from the ISP then you can assign to the phone and test.
One last thought is that via EManager in the CIX40 go to IP-Telephony->IPT Data and select the CA extension. Try changing parameter 17 "IP NAT/No Peer to Peer to "Enable" (default is "Disabled". Sometimes a phone behind a NATted firewall will have this issue. From what I understand this setting causes the IP card to handle the voice packets as an intermediary devices vs an IP to IP peer connection. I would try this one first since it is very easy and can always be put back.
Hopefully one of these proves to be the solution. These are some of the sloutions we have found when encountering this issue ~ Mike.

If an AT&T Uverse is involved then my solution was to set the DMZ in the router to route to the phone.
