I have an ipoffice with about 100 users. I have 1 person whose VM is turned off about every 7-10 days. I reset it to on and a week later it is off again. I can't find any timed events or COS differences.

At this same location I ALSO have 3 IP phones in one building out of 20 that lose their addressing so the VLAN needs to be reset about once a month.

Coincidence?

Is this VM Pro or Embedded. If VM Pro, do they use One-X Portal?

Change the Manager Administrator password.
My guess if it is not a bug, that someone might be leaving a copy of manager open, and makes changes to it.

I have never heard of this. If they are not on the most current maint release, i would upgrade to that first. If problem persists, i would escalate to Avaya.

Can the user accidentally turn her voicemail off?

Also the 3 IP phones that get stupid in the other building are on a switch with 5 others that never have this trouble. On just those 3 I reset the VLAN and login about once a month and it's fine.

Presently what is the current release?

It's odd that only these few problems persist. It's just weird.
