I had this problem before and, like an idiot, I didn't bother to write down the solution after I fixed it. I've since realized the amount of maintenance I've got to do on this IPK II and started taking notes.
The problem is I've got a CAP orbit with something on it but no way to access it. There is no trunk active. Simply a lit red CAP key that I can't clear. I have been unable to barge in (nothing happens), 770 clear ("CAMP Cancel" on LCD but to no other effect), feature + conf followed by CAP (get a click but dial tone continues and no other effect). I can set other extensions to monitor that CAP orbit and they too will see a red lit CAP key and exhibit all the same behaviors. When I push the CAP key I don't get the usual "Busy" with the trunk or ext it's busy with.
The user can make outbound calls. They cannot take inbound calls. The message waiting light flashes red and the Answer key flashes. If they hit the Answer key they get "ERROR!!!" on the LCD.
I can assign another CAP orbit to one of the other keys on the ext and the user can get inbound calls just fine then.
Any idea how to clear whatever it is that is stuck?

I have never had your problem so the only answer I have is to re-boot the system at your first chance.
How many extension on this system? Does each extension have their own CAP keys? How many CAP keys per phone? T1 or PRI?

Maybe delete the key programming on the phone and re-program.

The system is set to reboot every night.
Each extension does have its own CAP key. In fact, if I remove this CAP key from the ext and program the CAP key to another ext the problem moves with the program.
The extension initially had only one CAP key and that prevented inbound calls. Adding a second CAP key allowed inbounds then, though the first CAP key is still red.
I could simply assign a new CAP slot and forget about it, but that doesn't really solve the problem or tell me what to do when it happens again. I hate to just walk away from it.
I've had this problem with two different systems now. The two systems are now linked via KCCIS over IP. (Offices in two cities.) One has POTS lines and the other is plugged into some VOIP routers the cable company installed. The two systems actually couldn't be more different, but I've had the problem on both now.

That sounds like you have the same Cap Key assigned to different stations. Have you gone through all your stations to make sure you haven't double assigned that Cap Key? I have seen this and that was always the case.

I had this problem before with IPK II's K-CCIS using IAD cards. If the link went down & you tried to acces it, the CAP would "stick". NTAC said to upgrade the IAD's to PVA's. I never had a chance to do this before I left my old employer, the IT guy just got into the habit of assigning a new CAP to the extension it got stuck on. Of course, a re-boot of the CPU relesed it...

Just a follow-up:
I never did pin down the problem but figured out a treatment for the symptom.
Program 20-01-09
System Options > System Options > Camp-on Cancel Time
I set this to 300 seconds. We still have the problem from time to time but now it clears itself in 5 minutes.

This was an issue on 1.5x and lower software.
Are you running the last release of the IPKII software, 1.61?

I have a customer with similiar issues and when their PVA cards lock up, so do the CAP keys off some of the phones that happen to be on the call when it happened. I do like the idea of shortening up the Camp-On Cancel Time.
