I keep losing the PS19AC at a particular site. Mainly in the main cabinet. In one instance the power supply was sparking.
The system is in cooled server room behind APC battery backup. Other servers and equipment are plugged in with no issues.
The card list is as follows 3 16skd, 2 PRI, 8drcm PUGW and 16sh.

If I were a make a hunch I would look at your 16sh because they draw a LOT of power and that power supply is weak ESP on ring current.
